Lien Form #21 Effective January 1, 2009, the 6-Part 21 Lien Form will no longer be accepted to complete a lien. However, copies of this waiver of privilege form will still be accepted to release previously filed privileges. To date, no separate form submission guidelines are provided by the issuing service. As of 1.1.09, the 21 in 6-part privilege form will no longer be accepted in order to perfect a privilege. Any filing form that follows our template and contains all the information listed is acceptable. However, non-compliant forms will be rejected. (Please note that all information must be entered on the deposit forms, computer generated or printed legibly. Forms containing illegible information will not be accepted.) Four (4) identical copies of the completed Lien Collection Form must be made by the Lien Holder and filed with the Oklahoma Tax Commission (OTC) or engine license agent (MLA). After processing, one (1) copy will be attached to the title documentation along with a copy of the filing fee receipt. One (1) copy will be sent to OTC with a copy of the filing fee receipt in the agent`s semi-annual report. One (1) will be submitted to the ALM office. One (1) copy of the Lien Registration Form, one (1) copy of the Pledge Fee Receipt and the four (4) Partial Lien Waiver forms will be returned to the Lien Holder.
